2025-26 Athletic Participation Medical Clearance Information 

To prepare our student-athletes for the upcoming athletic seasons, the following items are required to clear students for participation:

*Students must have a valid WSISD student ID# prior to completion of forms.

The following items must be completed prior to participation in any try-out, workout, practice, or game:

1.UIL Physical & Medical History Form
All WSISD student-athletes entering the 7th through 12th grades must have an ANNUAL physical examination. The physical must be dated after May 1. 2025 for the 2025-26 school year. All physicals will expire at the end of each school year, despite the date of exam. Parents must upload a copy of their student's physical exam into Rank One for approval by the athletic training staff (see step 2 below). This must be completed on the current UIL Pre-Participation Physical Examination Form. 

2.Electronically submit the WSISD Athletic Participation Forms
This may be submitted at any time, regardless of when the physical is scheduled. The dates of the UIL Physical/Medical History Form and electronic form do not have to match. The remainder of the information will be submitted electronically by completing the forms on Rank One. Once this section has been submitted and accepted, you will receive a confirmation email for your records.

3. Complete Neurocognitive Performance Testing for Required Sports
White Settlement ISD requires Neurocognitive baseline testing, utilized as a tool in the concussion management protocol. High school athletes are required to complete baseline testing in the assigned years prior to participation in specified athletic activities (practices or games). Students who do not complete testing with their assigned teams at the end of each school year may schedule an individual testing time through the Sports Medicine Department. High school sports required to test prior to participation include: Football, Volleyball,Wrestling, Basketball, Soccer, Baseball, Softball, and Cheer. A STUDENT WILL NOT BE CONSIDERED CLEARED FOR ANY ACTIVITIES UNTIL ALL OF THE ITEMS HAVE BEEN SUBMITTED.
